I have the Canon S5. The lens adapter comes in 52 mm & 58 mm.

What difference does it make and which should I get?

It matters because teleconverters and wide-angle lenses (as well as filters) all come in different sizes. The Canon ones (and several other brands) are all 58mm. The Raynox ones (and several others) are 52mm.

You just need to match your adapter to the lenses you plan to buy.

If you're only planning to use filters or Raynox lenses, get the 52mm; you'll save money on the filters. Filters come in a lot of different sizes, but the larger the piece of glass, the more expensive.

Of course, you could buy one of each and then be covered for just about any lens that goes on a Canon S5.... :)
