Does everyone at table need to be on plan?

One couple doesn't want dining plan so can two couples be on and one pay cash?

I'd say the day someone at WDW says "We don't accept cash" is their last day there.

Yes, absolutely. The only caveat is that if you're all in the same room/under the same room reservation. If the dining plan is selected as part of the package EVERY PERSON on that reservation must have the dining plan.
