Does anyone have a form for this? COPYRIGHT transfer / release

Hi all,

I've been searching the web and coming up short, must be using the wrong terms?

I took some pictures on Monday for an organization, and need to give them copyrights to the photos. (Not just a print or use release--I am actually giving them the RAW files and allowing them rights to edit and publish the photos.)

Does anyone have a form / template that they know of, that I can sign for them? Ideally something that would allow for shared copyright (we agreed I could retain the rights to edit and display for my portfolio).

Yes I know, ideally the organization would have their own form they want to use, but they don't. This is a teeny little family business type operation. And yes I know it's not normal practice to assign over actual copyright, but that's what's happening here.

If no one has a shared-copyright type form, what about just one that transfers all rights (and then I could adapt it)? I can't even seem to find this.
