I haven't been to the California parks [YET], but my understanding is you will receive a "wheelchair return time" much like a FP. I believe you get this card at the ride entrance, though possibly the exit. You and your party will need to return within the window noted on that card. They may only allow up to 5 others with you (total of 6) if you have a large party. This is to prevent a back-up of people at the accessible entrance.

The accessible entrance is usually the exit, if you're not sure, just ask the CMs when you pass the entrance if it's accessible. They should be marked on the map, but the map can be confusing. Yes, your whole party goes with you. You may be asked to split up inside. For example, there are only so many companion seats in shows, so they may ask some people to sit in another row. You may or may not get a return time, it depends on how many guests need the accessible queue and how many the ride can accommodate. We got a return time on Pirates that was 2 hours when the regular line was about 20 minutes and we got another for less than 30 min for HMH when the line was over an hour.
ETA: The return times are handed out at the accessible entrance. There isn't really any way to know in advance that you'll get one. Sometimes there's a line, like on Peter Pan and Jungle Cruise, and the return time is actually to return to wait in that line. It really depends how many people are using that entrance at that time and how many parties the ride can accommodate at once.
