Disney Intro Video

I recently watched a Disney World home vacation video where the intro was the Walt Disney Productions intro customized with the family name. I thought it was really cool. I googled and tried to figure out how to create one for myself to no avail. I was wondering and hoping I could get some advice here on how to create one, or even led in the direction where I can have someone create one for me.
