DISigns help needed

I have just spent several hours going thru the creative threads and lots of Photo buckets but I have not been able to find what I am after. So I am turning to you guys for help! I am looking for a design for a new tattoo I have seen one I love whilst googling, but I can not find out how to make it. Can someone help I really love the idea and wish I knew how to do it.
