DDP items that cost extra credits

A couple of years ago my family went to WDW with a TS Dining plan. We ate at Rainforest @ DTD and decided to try the Volcano Sundae. We were told it would count as the desert credit for 3 of our meals. This wasn't a problem as we had plenty of people on the plan and it was more than enough to share. But this leads to my questions:

1. What items from parts of the menu that would normally be included (entree, drinks, deserts) are actually not, or cost multiple credits to order? If anyone has a comprehensive list of items and how many credits to get them I would love to see it.

2. If there weren't at least three adults dining, could we still have ordered the volcano (maybe using 2 credits and paying 1/3 the price)?

I know entire meals can be multiple credits i.e. 2TS credits get you 1 Signature meal or Dinner Show but I'm talking about individual items like the volcano I mentioned above or (as I would expect to make the list) the Kitchen Sink at B&C

1. The dining plan includes a non-alcoholic standard fountain drink such as coffee, tea, iced tea, soda, lemonade, basically beverages that come with free refills. There are many non-alcoholic beverages that are not automatically included. Some of the Disney owned restaurants will allow other options, but always ask if you're interested in milkshakes, smoothies, after dinner coffees, virgin cocktails - anything other than the standard beverages. Non-Disney owned restaurants will not include anything beyond what is allowed by the dining plan because they are only reimbursed by Disney according to the rules of the dining plan.

Most have said the Kitchen Sink is not included at Beaches & Cream, but a few have said they were able to pool credits, but this isn't something you can count on.

Anything that says "for 2" will require 2 diners' credits.

2. No, you cannot combine credits and cash for an item that requires multiple credits or any item "for 2."
 
Make it simple and don't get the dining plan.

At the time I was a CM and the plan was a ridiculously good deal (otherwise I probably wouldn't have, I've done the math and the way my family eats there is no way we could come out ahead paying full price for the DDP).
 

If you see an appetizer or entree marked "for two" then two people must order it.

Other than that, and the OP's note about Rainforest Cafe, there is no other pooling of credits allowed at this time for purchasing multi-serving items that I know of.

Via Napoli is mostly said to explicitly not allow the pooling of dining credits for larger pizzas but a few posters have said they were allowed to pool 2 adult credits for a large pizza - if there were only 2 adults dining - you're not allowed to pool 2 credits for a large pizza intending to feed more than 2. Don't count on being allowed to pool credits there, though.

Child credits won't work because they are only allowed for items from the children's menu.

They don't ever allow a combination of points and cash to pay for any one meal or item. Each person pays either with cash or with dining credits - not both.
