That is a good question! I am curious - I would guess it's yes.

We last used the plan in October 2007 and we did not have to pay the upcharge. We were prepared to, but we were not charged.

I have not read any reports that anyone else has been charged OOP for the upcharge, but it could be that they were not surprised so they just didn't report.

I would mentally prepare to pay the $2 and then be pleasantly surprised if it's covered by the plan.

Yeah, I have no problem paying a bit extra for steak if that is what the boy wants. He has some pretty grown up tastes and I think he would like that for a change.
