Ct Medicare united healthcare reward program

My parents have united healthcare (Medicare). They both received a rewards program brochure in which they would receive $40 prepaid Visa card each if they mailed in four completed forms. I went to all of their drs and got the forms signed. They both only received $10. I called UHF and they tell me I have to call credit co which in turn tells me to call UHF. Did anyone participate in this program and what results did you get

I work in healthcare. Not a big fan if Medicare replacements. More options with regular Medicare.
