Cruise documents- questions on filling them out!

OK. I see that everyone needs a Cruise Embarkation form, Bahamas Immigration card ans Cruise Contract Signature Form. Now, how about the Payment Authorization Form? I assume there is only the need for the actual credit card holder to fill it out right? Do yo really only list the names of those you want to have charging privileges or everyone in your group? I ask this because it says elsewhere that if you don't want some members of your party to have charging privileges you need to go to Guest Services onboard to remove names. Does that mean they put everyone on anyway? Lastly, what about the Departure Info card - must that be filled out and by everyone? Is that just for our information in case you get amnesia during the crusie or do they really collect that so thaey can contact /make arrangements should something unforseen happen to you during the cruise? If it is to be filled out, if your vehicle is not at the port but at a nearby hotel, should that be listed?

The charge authorization form is one per credit card. If you do not want children to charge then don't put them on this form. However, you will still need to go to the guest services on deck 3 to stop their onboard charging.

The departure info card is if something should happen that DCL would have to start making arrangement for you on their end....such as Hurricane Floyd when the ship went to Ft. Lauderdale or 9/11..., etc...
