Hello. We are going on the 2/19 2004 3 day Wonder & have received our cruise documents. We are a family of 5 - husband, wife & 3 children. How many "Payment Authorization" Forms do we need to complete? We don't want our kids to have charging priveleges. If my husband and I both want charging priveleges, do we complete 2 forms or just 1? What do we do with the forms that we don't complete? Do we throw them away or just leave them blank?

You only need to complete one form per cabin. If you don't want your kids to having charging priveleges, you have to go to guest services once on board and they will delete it from their KTTW card.
