Hi Yvette!

Sure, I'll be happy to give you the scoop, we stayed in a concierge room at the Grand Californian for a weekend in May. It was great!
I'm not sure how you're arriving, but when you pull into the guard gate area they will check your name and you will be given a parking pass that states you are a concierge guest. Just tell the bellhops upon arrival, and they will call a concierge staff member to meet you and escort you directly to the concierge floor...they did the check-in in our room!
The concierge lounge is a nice size and features continental breakfast, afternoon snacks, and a very nice evening wine/appetizer service. The may have late-night snacks too, we didn't check for those. The staff comes around and serves you drinks, unlike the concierge lounges I've experienced at WDW resorts. They were VERY nice, they even brought complimentary copies of "Disney Adventure" magazine for our 8-year-old to keep, which she loved.
I hope that helps a little! If you have any other questions, feel free to ask!
