I have read conflicting statements regarding refrigerators. Most have indicated that there are insulated boxes to keep items cold (with the help of ice). Is the same true for Cat 3 and above? On the room descriptions, they actually use the term refrigerator.

I've only been in Cat. 3 and have not seen what the other staterooms have. I have heard them described as 'beverage coolers.' That is a pretty accurate description of what we had also. We found it necessary to cool things off in the ice bucket before putting them in there. It kept things cold fairly well but could take quite a while to GET them that way. So, our collapsible cooler was indeed needed and used.
 
We've stayed in a Cat. 2 twice. It is not a true refrigerator. It's more of a cooler type box. If you put cold drinks in it they stay somewhat cool but not cold. We found it easier to have room steward put ice in the bar sink and keep our drinks in the sink. HTH.
