I'd call and check. A couple of years ago we had ADR's for 3 people and then one person decided to come on trip last minute. When we tried to add her to ADR they said we couldn't. We ended up making an ADR for somewhere else that night.
My MIL & FIL surprised us by showing up unannounced at the beginning January and stayed for 4 days. When we checked in to our ressies...we were able to add 2 people (total of 7!!) with no problems at every TS.
You shouldn't have any trouble adding a 4th person on. More than likely you would have been seated at a 4 person table anyway.
It depends on the restaurant. We had a reservation for 3 at O'hana last year and they could not make it 4. We ended up checking my 7-year old in at the Neverland Club and we were, indeed, sat at a table for 3.
