We are going to Kona for dinner during our trip. But, we would like to try the Tonga toast. So should I just make another reservation? Or should I just go to Captin Cook's and have them there?

Also where is Captin Cooks located?

My suggestion is why not make two trips to the Polynesian if you have the time. The 1st one can be for your ADR at Kona and the 2nd one for the Tonga Toast.

As for your other question Captin Cooks is located on the 1st floor of the GCH and it's just past the frame store on the right side, so you can't miss it.

FYI - Boatwright's at POR has something sort of like it - I think. I nver tried the version at Kona. Do skip the Strawberries and cream if you go there though.
