Can you still fax room requests to Dolphin?

I tried to pull up the form off of the GLO link to fax to the Dolphin for room requests, etc. When you click on the link it says the file can't be found - just wondering if the form was discontinued.

I would like to know this and also what kind of requests can you ask for? which are the better rooms / views?
