Hi. Has anyone ordered one lately? If so how, gotdid yous get the form(where) or from the internet?
DD16 needs one. The one she has is just a regerstration of birth and she needs the real thing to get her social insurance #.
DH lost his and needs a new one.
DD's boyfriend also lost his.
We are all heading to Disney(driving) the first part of March.
Father inlaw went on one site and it was over $70.00, then DD found another site for around $35.oo but had questions like -where your mother lived at birth, weight and so one. Hey its just a replacement.:confused3

I had to get my son and myself replacement birth certificates and my youngest daughter a new one last year. I went online, filled out the form online and just ordered it that way. I'm pretty sure that it was $35 for each one. And it didn't take a long time to arrive either. It was so easy to do it that way. And yes I did have to fill out the info on the doctor who delivered me. Thank goodness my mom remembered. Hope this helps.

I had to get my daughter's renewed this fall; it had expired. You can print off a form from their website and bring it in with all the necessary photos, or you can drop by a passport office to pick up a copy of the form.

My wallet was stolen in Vancouver this past summer- :scared1: But...I did the on-line aplications and everything came through OK. Prices depends on whether you need your long or short form birth certificate. As a back up call your mp and ask about when is the erliest possible date that you can get it. Sometimes they can get things done alot faster than if you do it on-line. :thumbsup2

Short form is where you get the little card that you must not laminate, and the long form is on a legal 8 X 14 piece of paper that contains all of your important info. My DH needed his long form for his Alaskian status. Most people only need their short form.:thumbsup2 Don't pay for it if you don't need it:)

Wow... were the heck was I when I wrote this??? :scared1: :scared1: The question was CLEARLY about birth certificates; and here I am, rattling on about passports. Oh dear, maybe I truly am losing my mind! :confused3
