Birth Certificate - what is a Long form?

I'm about to apply for Canadian Passport for my infant, since we will be going on Disney Cruise next year. I've read from this forum that Long Form is required?

When I applied for my child's birth certificate online, I have requested (and paid) both short and long forms. I received an envelope in the mail three weeks ago with the following:

1). Canada (Ontario) Birth Certificate - I believe that is the short form since it is quite small in size (rectangle shaped)

2). A long piece of yellow paper (basically a copy of Statement of Live Birth I've submitted) - called Certified A True Photostatic Print of a Record - Is this the LONG FORM?
 
The #2 you reference below is indeed the LONG form in Ontario.

Basically:
Long Form (Certified) - This contains all registered information, including:

Name (last name and given name(s))
Date of birth
Birthplace (city, town, village, municipality)
Sex
Location of birth (hospital, birthing centre, etc.)
Parents' names
Parents' dates of birth
Mother's mailing address at the time of birth
Duration of the pregnancy
Number of children born to this mother
Parents' birthplaces
Weight of the child at birth
Type of birth (single, multiple, etc.)
Attendant at the birth
Date of registration
Registration number
Date issued

It is provided in the form of a certified copy. You may need a certified copy if for example:
You are moving to another country
You are an executor for a foreign estate
You are adopting a child abroad
You are filling out certain citizenship or immigration documents

You only need the long form when your child is born out of wedlock... For example. My DD5 needed it as she was born 2 years before her mom and I tied the knot. However our DS1 does not require the long form as he was born after our marriage.

You would think the gov't would get with the times. Oh wait, stop another revenue base fee that would be stupid!:rotfl2:

I'm taking half-day off tomorrow for passport application & renewals (DD4 also needs renewal), so try to make sure I've everything they would need.
