We have spent the majority of our last three trips to DW at MK, and you seem to already have most of the tips I know! We consider it an absoulte necessity to get to the park at least 20 min before opening. We get most of Fantasyland and Toontown (or Tomorrowland depending on the day) in the first 2-3 hours in the parks. The park seems REALLY empty during that time, especially compared to about 2 o'clock at Fantasyland

it is packed then! We always hit Dumbo once or twice and then head to Pooh and ride it several times and then other rides - those are our DDs fav's so we ride them first so we can get more than one ride in without lines.
At toontown, we got to the princess/fairies meet as soon as it opened. Our DD wanted to meet princesses instead of fairies, and we found that princess line was MUCH shorter than the fairies line. There were about 15 of us waiting for princesses compared to about 75-100 waiting for fairies, and this was as soon as Toontown opened, so I would get there early because I can imagine that the wait only gets worse. We don't like lines, so we don't mind skipping things if they have a long line - it helps that our DDs are 3 and 1 and so there are very few "must dos" for them - they don't really know if we miss something! We also really like Mickey's Philharmagic - the girls love it and it is a nice place to cool off.
I know many people avoid parks on EMH days, but we always get park hopper passes so we do go to MK EMH in the morning, ride until it starts to get crowded, then head somewhere else. We found we could get alot done just during that hour of EMH and our girls are already up, so we figured why not? We also did MK EMH during the evening - somehow we were all still there and up for the second parade, and we found a place to watch it in Frontierland and there was seriously NO ONE around us at all. I don't know if this is normal or not, but we had a whole side of the rode to ourselves. We got alot of character interaction and it was wonderful!
Casey's has become our favorite CS in MK, not sure how it happened since all they have are hot dogs, but now Casey's just says "MK" to us. We love CP for table service.
TTA is great, especially if you just need a break for a few minutes. It is also a good way to pass the time if you have one child that doesn't want to ride something while the other does.
