A lot really depends on what you look for in a burger. Some people rave over more exotic burgers with loads of toppings and unusual sauces with little focus on the actual burger. I believe any burger needs to start with really good meat and you can build on that to whatever level you desire. I prefer basic and add only what I want, which is usually just ketchup.
The best burgers I have ever had in WDW are:
ESPN Club (quite a few years ago)
Le Cellier (before they went gourmet)
The Plaza (Magic Kingdom)
The Rainforest Cafe in DTD has a decent burger, too
The worst:
The new burger at Le Cellier
The snack bar CS at the Wilderness Lodge
Most of the burgers in WDW are pretty good and almost identical. The CS burgers can be almost as good as TS at half the cost, something to consider unless you really want the atmosphere of the TS restaurant.
