AP certificate voucher

Could someone tell me how to enter the info off of the AP certificate to get into the passholders page on the Disney World website.

The three letters come first in the roll down box, followed by the 3 digits after those letters. The next three boxes are the date that it was issued, example today would be 07 18 05. The last box is the tricky one. It is the last set of numbers before where the price is printed. It can be one to four digits. Use only the digits right next to each number. Do not use any before the space. For example my pass had 12 34 then the price. You only enter the 34 in the last box.
