Another passpot question

I am trying to fill out the kids and mine passport and i have a couple questions
On the kids it ask information of applicant which will be me but it asks for my surname(last name at birth) but this is different then mine know as i am married( i do have the same last name as my kids) does this matter??
Also on my application it also asks for my last name at birth but that is differnt then my last name know, it mentions that i might have to send in the original marriage license along with id showing my last name know and sigiture
is this honestly required?
I need to send them out by monday please help
Thanksoh also i know that i am not supposed to sign the kids and the kids don't sign there own( once they come in) but i have a 11 year old that has to sign his own does he have to sign it when it comes in also , and do we have to sign our own when they come in???

Yes, you will use your surname at birth. No, you won't have to send your marriage certificate. The form says that you might have to provide it. If there was a problem, they would contact you.
