If you mean do all of the kids wear princess costumes - no. Many will have costumes on, and many won't. It's pretty much a wear whatever you are wearing to the parks that day costume.
 
We've eaten there 3 times. There have always been a few kids dressed up, but most everyone is just in their regular "Day at Epcot" clothes.
 
We are all adults going. Will we feel out of place with all the princesses? I heard the food is really good and we are thinking of going on our trip next year.

You'll get all kinds of opinions on the food. I think it's fine to good. I think it's an excellent use of a TS credit if you're on the dining plan. If you're not, and you're not going because of princess-obsessed folks, I think there are better options in the WS if you're focusing on the food, specifically Chefs de France, Spice Road Table, or La Hacienda San Angel. Akershus is quite pricey if you're paying OOP.
 
We went last week because my DD11 asked to go to the castle and I said no. I picked Akershus instead. We did not have the dining plan, ended up being $290 with the tip (3 adults, 1 kid, a couple of alcoholic beverages). My daughter enjoyed meeting princesses but I wished we did just about anything else for that much money. Food was good, but not great. There are many other restaurants I think we would have been happier with - and those probably wouldn't have cost that much.

My daughter wore shorts and a tank top, with her Ariel ear band. She no longer has any princess dresses that fit, and the "feels like" temperature was 100 degrees. I didn't feel out of place in normal hot-weather park clothes.

Did you say no because of the price? Because I just prepaid our castle meal (breakfast) for 3 Disney adults, 1 kid and it was less than that and we'll have a yummy breakfast. I did not enjoy the food at Akershus (we did dinner). Just a thought for next time...

Have you actually been there for breakfast though? I thought the breakfast food we had at CRT was worse than anything else I have eaten on Disney property. Whereas I have never had a bad meal at Akershus. I guess Disney dining is never consistent.

$290 was the total including alcohol (2 mixed drinks for my husband, 1 wine for me). Our meal was dinner, not breakfast. We've been to CRT at least 5 times, so we definitely didn't "need" to go. I also booked and planned the entire trip less than 2 months in advance so I also didn't want to try to find a reservation for CRT.

The meal was nice, my daughter enjoyed meeting princesses, but I really wished we saved some money and went to la Hacienda or Tutto Italia.
