ADR, Party of 4...or 5?

Not sure when booking my ADR's if my two year old is included. She will be sitting in a high chair, and not on the dining plan....

Yes, she is included in reservations.

I think of it this way... would you need the space at the table for a high chair? The answer is yes. High chairs take the place of a regular chair. So if you don't book accounting for this, you won't have a spot for the high chair.
