A question for those who have rented/traded points

Don't know if this has been posted earlier (or if this is the appropriate forum...if not, sorry), but I am curious if anyone has a simple document that they use for these transactions that they are willing to share.

I know most people have the best intentions, but a little documentation never hurts either
