Did you log into your Saveology account? The form to fill out is there. Then you'll get the gift card in the mail.

Logged on to my (and my DH's) Savology acct. and got my voucher #'s and it gives the instructions to go to a Landry's page that has a place to enter the voucher number and you're mailing info (and email you used when buying).
Then I got the email saying that my gift card would arive soon! Easy-peasy!

I just ordered one and it said that it has an expiration date of Nov. 4, 2011??? We're going over Thanksgiving so I really hope this is just something Saveology states. The gift cards I've always received from MyPoints have never had an expiration date.

That's the expiration for the voucher, not the gift card.
