1900 Park Fare does a nice job with the birthdays...for DH's birthday last May, they brought the special Mickey cupcake (though that's total overkill since it's a buffett with lots of desserts, lol.) They also brought the autographed placemat (it's plastic-coated, so a "keeper") And of course, Cinderella, the Prince, and the steps all visit each table so you'll get to take photos with each character, too.
If you're staying at a Disney hotel, ask for the Birthday pin when you check in - they'll write his name on the pin with a sharpie and he'll hear "Happy Birthday" all day.
