They want the signature of whatever adult is assuming responsibility (for charges) for a minor child, or for a person not competent to sign for his/herself.

It means that's where the responsible adult should sign on behalf of a child or anyone not competent to sign (let's say your grown child who is mentally disabled).

So if you're the responsible adult (parent, grandparent, etc.) you would sign your name there to say that you agree to be responsible for those unable to be responsible for themselves.
