Depends on the type of tag you use.
If you have "window" tags, I recommend typing up a slip with your last name and first initial, and the name, city and phone number of your resort. Underneath that, put another slip with your business address and a phone number where you can be reached easily. Once you and your luggage arrive at the resort, take the top slip out and throw it away.
If you use permanent luggage tags (the laminated kind) then I recommend the names of anyone who might carry it, and a cell phone number. In addition, the back side should note that your itinerary is in the outer pocket of the bag. Then actually PUT a piece of paper with the itinerary (with phone numbers) in the outer zipper pocket (in addition to the copy that is inside the bag for damage emergencies.) Laminating a business card works well, too.
It isn't a good idea to use your home address and phone on a luggage tag; an unscrupulous worker could note this and know just exactly what house to rob and what number to call to make sure that no one is home.
BTW, when I made my new laminated tags, I used bright yellow paper and added a little stripe of hologram gift ribbon along the edge on both sides. It looks kind of flashy and silly, but that ribbon reflects light very brightly, and I can now easily find my case on the carousel because of the sparkle effect bouncing off the tag.
