Just an update. My mom requested the card on October 1st and it arrived in the mail yesterday, October 10th.
It looks a lot like a credit card, except the account number is just printed on the card and not embossed on the card. It does have the 3-digit security code (or whatever that's called) appearing on the back of the card after the account number. It says you can use it online for Disney catalog orders, so I don't know if you'll have to enter the 3-digit code then or not (I've never ordered from Disney online, but I have ordered from other online merchants that will ask for the 3-digit code from your credit card).
There is the white signature area on the back of the card, like on a credit card, but nowhere does it say that it has to be signed. She didn't call to activate it while I was there, so I don't know if they tell you anything about signing then. I don't think her name appears on the front of the card.
I think that's all the interesting things I can think of right now.
It does come with a small pamphlet that summarizes some of the program information. And there is other info printed on the back of the form the card comes attached to. Mostly some FAQ-type stuff. But still nothing specific about the actual places you can use it--just the usual vauge language about most places and most merchandise.
