Tipping/concierge?

This is a first for me. Please educate me. How, when, where and what to tip for. I know about tipping Mousekeeping, bellman and valet, but what about turndown service and such.
Also, How much?

Oddly I have had tips turned down more than accepted. Although I only have one experience to draw from...
