Teacher Portfolio Question

I am trying to get a library aide job and have been told by several people in school systems around here that I need to get together a portfolio of my MLIS school work, similar to what education students do before they graduate.

So just a basic question: for projects that I did with a group--I feel want to make sure that my portion of the project is understood and not appear to take credit entirely. For example, if I did the powerpoint for a grant proposal project that was done with 3 other people, I would like to include the slides (they are my work) but I don't want to represent that I completed the application that is shown in the screen caps.

How do you handle it? Just label that first powerpoint slide with all our names? A footnote? I just want to be honest!!

Please let me know your thoughts.
