I have a set of twin teenagers (18) who are physically and developmentally delayed, very small for their age, and have "kid" tastes. Do the restaurants typically allow for accommodations such as allowing ordering from the Kids' menu, splitting an adult meal?

We would not be using the Disney dining plan.

Disney owned/operated restaurants will allow anyone to order off the kids menu. It’s hit and miss otherwise, but the good news is in the parks nearly every restaurant is owned/operated by Disney- except for some in Epcot. Quick service will be no issue.
 
Above is correct. Just be aware that you may be charged for an adult-sized portion of the kids’ meal.
