I am never around for the waitress to ask me about change or a tip. Do you not pay for the meal and either write the tip on the receipt at the table and leave or when cash just leave the tip in cash on the table and leave?

I had put money in the holder with the receipt by the time she brought the dessert out. She came back within a few minutes. But yes most places usually work like what you wrote. Unless its a place with a checkout desk towards the exit.
