I don't say anything, to anyone when they sneeze. Ever since I learned the origin of the phrase, I just never have.
 

Maybe just me but always thought of 'gesundheit' as an example of onomatopoeia............

I usually say "Bless you" or "God Bless."

If the person I'm with sneezes more than once I'll (jokingly) tell them that after 3 they get no more blessings from me.

If the person is someone I know really well they might get "ew" or "stop" or "oh my goooooddddddddd."
 
This fall has been a very sneezing, sniffling, coughing, and unfortunately, barfing, kind of semester. My "bless you"s have been followed up with a "go blow your nose, please" more and more over the last few weeks. I think the only kid who hasn't been sick is the one student who wears a mask on a daily basis. :)

If I sneeze more than once my husband says to get that shirt under control lol

I use whatever expression comes to my mind first with whomever I’m with. However it was an in-house tradition between the man and me to say:
SHUDDUP !! on the third sneeze. Those were the days 😊.
 
I haven't heard anyone day Gesundheit in decades.

 
Is it wrong to say nothing? It's a normal bodily function. The bless you part comes from a belief centuries ago when a person sneezed their heart stopped and it was the blessing requested to God that the heart started up again to live another day.

So I say nothing. I wouldn't make a comment if someone burped or passed gas, also bodily functions, so why would we draw attention to a sneeze, and why wouldn't we then do it when someone coughed.
 
I typically say nothing. Too many folks express annoyance around the bless you part that I've gotten away from saying it at all.
