I know it is smart but does DH have to wear a jacket and/or tie?

Not that i am aware of - i believe they are business casual. We used to go all the time to another one of their locations in Florida and never had to bring a jacket. Certainly not a tie. I think the only jacket in the World is V&A.
 
Jacket and tie? Goodness no!!
It's not a Disney restaurant, but I believe the Disney version of business casual would apply. WE haven't done Shula's for quite awhile, but we did Blue Zoo seceral times recently. Dress there was business casual, and that now includes shorts and t-shirts.

The food was good.
Service wasn't Disney friendly though--more "stern/correct".
 
Any chance of getting mushrooms as a side order, as we love mushrooms with steak?

I think business casual is perfectly fine for Shula's. The food there is really good (and pricey). I cannot remember if they have mushrooms to go with your steaks, I think they do but am not 100%. It is a neat place, in my opinion, I love the menu - it is a football!
