Questions re: documentation forms

I need help filling out the documentation forms that are sent prior to the cruise.
1)On the paper titled "Welcome to the Islands of the Bahamas" - For intended night stay, do I put 0 since I'm not actually spending the night there? Also type of accomodations? And hotel name or intended address? What do I put in each of those? City where I boarded - I thought I put Port Canaveral but it doesn't fit in the boxes allowed.
2)In the travel booklet - I have my 2 DS's traveling w/ me - in their books, where it asks if you are traveling w/ minor children, for them what do I put? On the contract signature form, do my DS's sign it under guest signature & I sign under responsible adult or I just sign leave the left hand side blank?
Thank you so much for your help - we leave on 5/26 Wonder for our first ever cruise & can't wait!!!

I put 0 for intended length of stay, Pt Canaveral and type of accomodations Cruise Ship. The rest I left blank.

I signed for both of my children. My kids did not sign at all. I put that they were travelling with minor children.

We had no problems when the CM looked over our paperwork. HTH
