If DH and I order something from Counter Service restaurant and we order chicken nuggets or pizza for DD, who is under 2, we have to pay OOP for her?

Yes. She can share, but if you order anything specifically for her, there could be a charge for it.

Some places, usually the TS, may provide something for her, but you will have to pay for things sometimes.

Yes. Children under the age of three are not included on the plan. At CS restaurants, they are to either share from your plate or you may order them something OOP.
