QR code for gift tags

Has anyone done QR codes for gift tags. Attached a picture or song to QR code. If so can you give me some easy directions?

You can download a QR app, I've used it to link to song on Spotify (making anniversary gifts linking back to a first dance song). Not sure how you would link to something not internet based.
