Planning memorable trip for Grandma's 92nd BDay..help

My Grandmother, aka "Mama Burns" accompanied me on my first WDW trip in 1979 or 80 when I was a little girl and she went with my husband, children and I in 2002 for Spring Break. She was not able to make the last trip that we took in March of this year because she had to undergo radiation treatment for 2 little skin cancers. She is just as spry as can be and keeps saying that she would love to go back....I tried talking her into the Disney Cruise but she doesnt like the water.
I would love to take "Burns" to WDW for her 92nd birthday in April, 2008. Other than my children, she is the absolute joy of my life and I want to make it extra special!
Her favorite ride is It's a Small World but she really enjoyed the Buzz Lightyear ride as well....she'll get on anything that the wheelchair will go on! She still walks but it is much easier to get her around quicker in a wheelchair!

If I can save enough money, I want to stay at the Grand Floridian or the Poly....input on these are appreciated.
Of course, my kiddos will be going...ages will be Daughter 15/Son 9/Daughter 5. She lives and breathes her Great-Grandchildren...so anything that she can do with them is great!
Can anyone give me helpful info of planning and tips to make sure that this is the best time of her life!?! How soon can I make certain reservations, etc.

You can start now if you want. What a great idea for her. Just be aware it will be super busy at that time of year, so hard to get around with her wheelchair. But if it doesn't bother her go for it.

We just stayed at the GF and given the theme and atmosphere, and your grandmother's age, I would pick there. It would be like stepping back in time for her. They are phenominal about doing special things for birthdays. The GF bakery can make some really beautiful specialty cakes. It will also give you such easy access to MK and Epcot.

Find a really nice restaurant to have her b-day dinner at like Victoria and Alberts (if you can scrape up enough for that too). The Hoop-Dee-Doo would be fun too, although a little trickier to get to. Make sure to get her a birthday button, so everyone knows it's her birthday. Then just take her to all of her favorite rides and some of the new shows like Nemo the Musical, which is amazing.
