Package?

I know you have to stay in a Universal hotel in order to receive the front of the line access, but do you have to book a package or can you just book the room? I was considering buying the $87 tickets for 7 days, but will hold off if I will need to book a package.

As long as you are staying onsite no matter what way you booked it,your room key is your Fastpass.
If you book a package it does not qualify as a stay on your Loews First account-apparently. Why not book the room through Loews using Supersaver rate or AAA as you will get it a bit cheaper, and like you said buy the $86 7 day tickets
