Need help creating a personalized door magnet.

Hello all, I need someones help. I wanted to make a personalized door magnet for our 2016 Disney cruise but i have no idea how to do it. I would like something that said Pera's family vacation 2016, and that contained our names on it which are Diana, Liliana, Joanna, and Esteban.
Can any one help me out with a printable design, Thank you.
