Do you any of you kind ladies and gents have a link to a name change checklist or one you made that you could email a copy of to me so I can pass it on to my brother for his GF. I guess he's getting hitched and didn't bother to tell me.

It's very simple, an article I read said it best:
"get your hands on that license!"
Once you (or your brother in this case) get your notarized copy of the license, take it to the Social Security Administration. You can do this by mail but every newly married bride I've spoken to said going in person is best because you have to also give them proof of who you are, like your driver's license, and it has to be an original, not a copy. You don't really want to mail away your DL. So, take the marriage license and driver's license and go to the SS administration office. You'll get your new SS card in a few weeks, then go to the Drivers License bureau, sign a new W2 at work and have work HR get your new medical cards ordered, and then call credit cards, cell phone, magazine subscriptions, etc. - many of these let you change your name on-line too.

No one asked for proof except SS and DL and work kept a copy of the license on file.

Hope this is helpful!

On the weddingchannel there is a section that gives some info. I pasted a little here but then it depends on the state.

Here is a website that sells a name changing CD www.capturedmomentsboutique.com.

NAME CHANGE

Legal records, including Social Security and your Drivers License. Find information about changing your name with Social Security.

Financial records, including your checking and savings account, loans and leases, and charge card accounts.

Professional items, including business cards and health insurance.

Personal items, such as your passport, personal memberships, and phone book listings.

Thanks gals. It hasn't been that long ago I did mine but I couldn't find checklist to save my life. The State I assume is/will be Michigan. I agree on going to SS office it is much quicker this way.

Here's the list I used. Wedding Checklist I think the site also sells a kit but I just used the free list on that page. It worked out really well for me, I printed it out then wrote all of my notes from all of my calls on it so I had a record of all my changes in one spot. That way I didn't call the same company 3 times and miss someone else entirely. I still occasionally get a few things I missed, or require a letter (5 months and I still haven't gotten that far), but most did it over the phone no problems.
