While looking for my HSN ad, I found this ad in the same place -- click on Earn Points, and then hit Refresh until you see if show up under Great Offers.

There is a 3 page health-related survey that results in a list of vitamins that the good doctor orders.... There is a pop-up for the 25 points.

Not sure if it will work for me or not. Says you get the points in 30 business days.

Since I'm a SAHM, I tell them my occupation is "Educator" all the time. Otherwise I never qualify for any of their offers.

Just one note. I did this survey awhile back and you have to join the mailing list. I haven't been able to get off the mailing list now that I joined.
