Lost Annual Pass?

If someone was, ah, foolish enough to lose their Dream Annual Pass, does anyone know who they'd need to contact to get a replacement, or indeed if it's possible to?

I'd contact the Annual Passport Bureau as they would want to alert the gate in case anyone else was trying to use it.
I'm sure that if you still have months left that you would be able to get a replacement, although may have to pay some administration fee
