Looking Trip Templates

Hi All, Have not been on this tread for awhile. Anyway I used to have great trip templates, but cant seem to find them anymore(might have deleted by mistake) I was wondering if anyone has any and can point me in the that direction. I would like them by park including Disney Springs and maybe a arrival and departure day too. What to have park hours, FP times, ADR times and place for notes would be great. Any help would be wondering.
