Looking for ID tag

Hi Everyone,

I'm looking for an ID tag that I could swear I just saw it the other day, but now I can't find it. The design has the castle on it and saying "what will you celebrate" and I think it has space to write/type in your name, address, phone #...

If I am totally wrong and just making things up...does anyone know where I could find something like this? It is for my daughter to wear around her neck...
