If I go to Kona for breakfast, will a side of fruit cup be considered an appetizer?

you can ask - some places/waiters have been known to allow it, but in general you do not get an actual appetizer with breakfast -- you get the entree (or combo platter) & two drinks (like coffee & orange juice.) At Kona, we were allowed to get the pressed pot coffee instead of two drinks (the individual press pots are usually about $7.99 each; makes about 2 large mugs of coffee.) DH had the steak & eggs platter which was excellent, I had the Big Kahuna platter...outstanding. Pics in January dining report, post #27 - link below.

Only dinner gets an appetizer. You can ask, but generally anything besides the meal and drink will be charged separately. If there's something in the meal you're ordering that you don't particularly care for, they would probably be willing to substitute it for fruit, so you wouldn't have to pay extra for it.
