I have eye prescription that needs to kept cold and trying to figure out the easiest way to get through TSA. Should I put it in a yeti type cup with ice- dump the ice go though security and get another cup of ice on the other side of security or pack a cold pack in the cup? I have a letter from my dr saying that it is prescription medicine and it needs to be kept cold

You can use a cooler with an ice pack.

How cold does it need to be kept. I take insulin and use a Frio like this. https://www.frioinsulincoolingcase.com/product/individual-pen-wallet/
They offer various sizes. If one of those won't work I'd agree with a small cooler and an ice pack.
To your initial idea, if the ice is frozen you're allowed to take it through, you would need to dump out any water before security though.

I have a cooling bag that I bought off ebay for my insulin pens. It has 3 small ice packs that go in it to keep things cool.

The TSA won’t care about your doctor’s letter. The ease of forging one is why.

Most insulin can be kept at room temperature, not FL heat, for about a month. After the month unused insulin should be discarded

Verify the safe temperature range that "cold" is. It is possible that keeping it on ice will be too cold and you may need to consider an option like a FRIO.

You can cheaply do it in one of two ways, assuming that packing on ice won't make it too cold. You can put it in a small insulated container with bagged regular ice, dump out the ice just before the checkpoint, then go to an airside restaurant and get more ice to fill the bag, or, you can bring "ice" that is actually a solid; frozen peas or frozen grapes are the most commonly-recommended options because the individual bits are round and easily conform to the shape of the container. (bags of other kids of frozen fruits or vegetables work as well.)
