I've been upgraded a few times.
The most recent one was after a big fiasco with the dining plan, so it was kind of compensation. I didn't pay for the upgrade or request it (or any other type of extra compensation), but I lost a bit of time getting the problem fixed.
So I would throw out that sometimes if there is a problem, free upgrades might be offered as a way to make up for something Disney has done wrong.
There have been other times when we've had problems with our room that we were not given any upgrades either (a toilet that didn't flush, a dirty bathroom). So it all depends.
I'd never request a free upgrade. I agree that if you want a certain room, book it.
